  • GP Referrals for ADHD & Autism Assessments

Our clinic provides comprehensive assessments for both adults and children who may have ADHD or autism. Referrals are accepted under the NHS Right to Choose pathway, giving patients access to timely assessments outside of standard NHS waiting lists.

By making a referral, you are enabling your patient to access high-quality diagnostic services delivered by experienced clinicians. The referral process is straightforward and designed to fit smoothly into GP practice workflows.

Who can make a referral?
Referrals must be made by the patient’s registered GP. This ensures we have the correct clinical information and allows us to communicate securely with the practice if additional details are required.

What you’ll need to complete the referral
  • Patient’s personal details (name, date of birth, contact information, NHS number if available)
  • GP practice information (including ODS code)
  • Relevant background information on ADHD or autism concerns
  • Details of any current mental health treatment or safeguarding involvement
  • Copies of any supporting reports (e.g. from paediatrics, speech and language therapy, occupational therapy)
Data protection and confidentiality
All information submitted through this referral form is handled securely in line with NHS data handling requirements and GDPR. Data is encrypted, stored securely, and accessible only to authorised members of our clinical team.

What happens next?
Once we receive your referral:
  1. Our team will review the information provided.
  2. We'll contact the patient (or their guardian if under 18) to explain the next steps.
  3. You will receive confirmation that the referral has been logged.
